Billing & Insurance
At Wozani Integrated Health, we offer flexible billing options to suit your needs. Most of our patients use their traditional health insurance billing, and we accept all major insurances. For those who are self-insured or uninsured, our Direct Primary Care membership provides comprehensive primary care services for a monthly fee and small per visit copay.
Additionally, we offer a transparent and reasonable fee-for-service option for patients who prefer to pay per visit or procedure. Choose the billing method that works best for you and receive the quality care you deserve.
Health Insurance
We can bill all major insurances (including Medicaid). We realize that not having the ability to leverage current health insurance can be a barrier for some of our patients. If you choose to use your insurance, visits will be billed based on current Medicare rates.

While we accept most major insurances, we may not be contracted with your specific plan. It is the responsibility of the patient to confirm if we are contracted with his/her plan.
All copayments and unpaid balances are expected to be paid at the time of service. Self-pay patients are expected to pay in full at the time of service.
Direct Primary Care
Direct primary care (DPC) is a healthcare model that directly connects patients and their primary care providers (PCPs) through a membership-based arrangement. Unlike traditional fee-for-service models, DPC eliminates the involvement of third-party insurance companies.

While most of our patients use traditional health insurance, our Direct Primary Care membership may be the best option if you are self-insured or uninsured (*This is not a health insurance plan). With this option, for a monthly fee and small per visit copay we will manage all of your primary care needs including your annual wellness exam or physical.
New Patient Visit ($225)
Initial comprehensive consultation with new patients. Discuss current health concerns and develop a treatment plan. [60 minutes]
Telehealth Appointment
Life keeps moving, and so should your wellness journey. Integrative healthcare through telehealth appointments for existing patients is a convenient option for follow up and some sick visits. [30-45 minutes]
Comprehensive Follow-up ($125)
Targeted physical exam to address new concerns as well as follow up on current medications/supplements and plan of care. [~30-45 minutes]
Travel Medicine Consult ($75)
Embark on your journey with confidence and peace of mind. Travel Medicine consult provides personalized guidance and care to ensure your health and safety abroad.
Sick Visit / Urgent Care($75)
Targeted physical exam to address acute injury or illness.

Sports Physicals ($60)
